MAY 20th: Citizens will cease honoring checks and debit card transactions effective May 20th. All contributions should be directed to Avidia Bank for processing. Any unused checks and debit cards from your Citizens account should be destroyed on May 20, 2016. Neither debit cards nor checks from Citizens Bank will be useable or honored after that date.
May 21st and 22nd: HSA Debit Card - blackout period. Please plan accordingly or you have the option to be reimbursed after May 23rd.

Provide your new account number to any entity that you’ve authorized to perform a direct withdrawal from your HSA account. They need to be notified immediately of your new routing number, account number and/or debit card number.
Activate your new Avidia Bank HSA Debit Card by calling 855.472.9399; and selecting option 1.
If your employer is currently remitting payroll deductions for contributions into your Health Savings Account, please refer to the Avidia Bank Health Savings Authorization for Payroll Deduction Form that has the Avidia Bank Routing Number and Bank Information pre-populated. Complete the form and send it to your Human Resource Department to insure your deposits have no interruptions.
Use only your Avidia Bank HSA debit card. Your HSA account balance and account information can be viewed using Avidia’s online banking.
